We strive to see you at the time of your appointment and not waste valuable time filling out paperwork.
- Please print all forms, complete, and bring them to your first appointment. We will need to photocopy your insurance card, and photocopy your drivers license, please make sure you have these items with you.
- If you are unsure if your Insurance covers Chiropractic coverage, (Individual plans purchased by you DO NOT have chiropractic, Covered California Plans DO NOT have Chiropractic as a benefit), please print the Insurance Coverage Worksheet and contact your Insurance Company.
- If this is for a minor appointment (18 and younger), please print the Minor Release Form as well.
If you are unable to do this, please arrive 15 minutes prior to your appointment time to complete your paperwork.
